In Pennsylvania, Medicaid goes by a different name: Medical Assistance (MA). For older adults and adults with physical disabilities, long-term services and supports are delivered through Community HealthChoices (CHC), a mandatory managed-care program run by the DHS Office of Long-Term Living.

CHC operates statewide through three managed-care organizations — AmeriHealth Caritas, PA Health & Wellness, and UPMC Community HealthChoices. It's designed for adults 21 and older who are either dually eligible for Medicare and Medicaid, or who meet the state's nursing-facility-level-of-care criteria along with the applicable waiver or facility conditions.
