Pennsylvania Home Care Medicaid

Community HealthChoices doesn't just cover facility care — it also delivers home- and community-based services for people who need a nursing-facility level of care but want to stay in the community. That list includes personal assistance, home modifications, personal emergency response systems, respite care, residential habilitation, community integration, adult daily living services, and participant-directed goods and services.

All of this runs through the same three statewide managed-care organizations that administer facility-based CHC coverage. Outside of Medicaid entirely, Pennsylvania also offers OPTIONS, a state-funded program for qualifying adults 60 and older that doesn't require meeting an income-eligibility threshold, though it can carry a sliding-scale co-payment.
