For Hoosiers age 60 and older, Indiana PathWays for Aging is the managed program that delivers long-term services and supports in the home and community. CMS lists the underlying HCBS waiver as effective July 1, 2024 and approved through June 30, 2029.
PathWays HCBS effectively took over from the former Aged and Disabled Waiver for this age group, and the published program description covers a range of service settings — a participant's own home, a loved one's home, adult family care, and assisted living among them.
The catch that families need to plan around is capacity. Waiver slots are federally capped, and when demand outpaces available capacity, a waiting list can form even for someone who otherwise qualifies clinically and financially.
