New Hampshire's Medicaid program, referred to simply as NH Medicaid, sits within the Department of Health and Human Services. What's easy to overlook is that DHHS runs a distinct medical-eligibility determination specifically for people seeking Medicaid long-term-care services — this is a separate process from filing the financial application, not a formality tucked inside it.

For older adults and adults with disabilities, the two main long-term-services settings are Medicaid nursing-facility care and home- and community-based services. On the community side, New Hampshire's key program is Choices for Independence, commonly shortened to CFI — a section 1915(c) home- and community-based services waiver designed for people who would otherwise need a nursing-facility level of care.
