The Choices for Independence program — CFI — is New Hampshire's flagship section 1915(c) home- and community-based services waiver, built for older adults and adults with disabilities who are at risk of needing institutional care. CMS's current federal record shows the CFI waiver approved effective July 1, 2022, running through June 30, 2027.

CFI operates within New Hampshire DHHS and is designed around a specific goal: letting someone who needs a nursing-facility level of care actually receive that level of support in their own home or another qualifying community setting instead. Because enrollment is capacity-limited, it's worth checking current wait times or availability rather than assuming enrollment is automatic once someone qualifies medically and financially.
