Once someone is approved for Michigan nursing-facility Medicaid, the state's Bridges Eligibility Manual (BEM 546) calculates what the resident actually owes each month using a post-eligibility formula: total income minus total need.
"Total need" is more layered than it sounds. It can include the personal-needs allowance, a home-maintenance disregard, an income allowance for a spouse remaining in the community, allowances for dependent family members, health-insurance premiums, and certain guardianship or conservator expenses — meaning the final patient-pay number is rarely just gross income minus the facility bill.
Current personal-needs allowances under this framework are $60 per month generally, $90 per month for a veteran receiving VA Improved Pension, and $37 per month for residents who are also SSI-eligible.
