Michigan spreads long-term care funding across several distinct programs rather than folding everything into one. The Michigan Department of Health and Human Services administers all of them, but which one applies depends on the setting: nursing-facility Medicaid covers institutional care, MI Choice serves people at home or in the community, and PACE blends Medicare and Medicaid financing for participants enrolled with a PACE organization.
MI Choice itself runs as a federal 1915(b)/(c) waiver, operated through regional waiver agencies that function as Prepaid Ambulatory Health Plans. Those agencies handle the practical work of intake, assessment, and care coordination on the ground.
