Any Iowa long-term-care plan has to begin with the income-cap math. Iowa HHS lists the 2026 threshold — 300% of the SSI rate — at $2,982 per month for the relevant long-term-care group. Someone above that number generally needs a properly structured Medical Assistance Income Trust, also known as a Miller Trust, and Iowa HHS is specific about the requirements: the trust must include a state-payback provision, and only the beneficiary's own income can be assigned into and deposited in it.

There's a second guardrail that's easy to miss: Iowa's current guidance on these trusts caps total gross monthly income at 125% of the statewide average charge for the relevant facility type or level of care. For nursing facilities specifically, that 125% figure for 2026-27 works out to $12,002.50 per month. Because that ceiling exists, trust drafting and funding really need to happen before the income actually becomes available — trying to fix a problem after a denial is a much harder position to be in.
