One thing Iowa families should know before assuming a common planning tool is available: the state doesn't currently have an enacted transfer-on-death deed for real property. A bill proposing one has been introduced in the legislature, but as of now it remains just that — introduced, not law.

- Expanded estate-recovery definition: Iowa's recovery policy explicitly reaches jointly held property, trust interests, and retained life estates — a broader net than an ordinary probate-only estate.
- Elderly Waiver stays separate: even as Iowa consolidates six diagnosis-based waivers into new combined programs, it deliberately kept the Elderly Waiver on its own track after hearing concerns about access for people 65 and older.
- Dual income-cap guardrails for trusts: beyond the basic income limit, Iowa's Medical Assistance Income Trust guidance adds a second ceiling — gross monthly income can't exceed 125% of the statewide average facility charge.
