For older adults and people with physical disabilities, Diamond State Health Plan Plus (DSHP-Plus) is Delaware's mandatory managed-care route to long-term services and supports. It's designed to cover both nursing-facility care and home- and community-based services aimed at preventing or delaying a nursing-home admission whenever possible.

Delaware consolidated most of its home- and community-based waiver programs into DSHP-Plus effective April 1, 2012 — everything except the waiver run by the Division of Developmental Disabilities Services (which today operates as the DDDS Lifespan Waiver). The programs folded into DSHP-Plus at that time included the elderly-and-disabled waiver, the HIV/AIDS waiver, the assisted-living waiver, and the acquired-brain-injury Medicaid waiver.
