Alaska applies a standard 60-month, or five-year, look-back to Nursing Home Medicaid and HCBS waiver applicants, counted immediately before the application. Under the current eligibility guide, a transfer made for less than fair market value during that window can lead to a period of Medicaid ineligibility — though notably, the ABD Medicaid pathway doesn't use this same long-term-care look-back.

A 2025 Alaska elder-law presentation, citing ADLTC 554D, describes the review as reaching back five years from whichever comes later: institutionalization or the LTC Medicaid application date. It also notes the state presumes that any transfer made by the applicant or their spouse was intended to establish eligibility, maintain it, or avoid recovery — unless the client rebuts that presumption with convincing evidence to the contrary.
