Alaska Home Care Medicaid

Alaska's Department of Health operates five Medicaid Home and Community Based Services waivers: Alaskans Living Independently (ALI), Alaskans with Physical and Developmental Disabilities (APDD), Children with Complex Medical Conditions (CCMC), Intellectual and Developmental Disabilities (IDD), and Individualized Supports (ISW). Each waiver serves people who meet an institutional level of care but who receive their services in a home or community setting instead of inside a facility.

For older adults specifically, ALI is the most directly relevant program. CMS describes it as serving people age 65 and older along with adults ages 21 through 64 who have physical disabilities and meet a nursing-facility level of care, while the state's own page frames ALI as available to adults 21 and older who need nursing-facility-level care. The IDD and ISW waivers, by contrast, carry their own stated waitlist or participant-limit rules that families should ask about directly.
