Virginia Home Care Medicaid

Virginia's main home- and community-based route for older adults and people with physical disabilities is the Commonwealth Coordinated Care Plus Waiver, known as CCC Plus. DMAS describes it as serving older adults, people with physical disabilities, and people with chronic illness or severe impairment who would otherwise need nursing-facility admission, a prolonged hospital stay, or specialized nursing-facility care. Both Medicaid eligibility and LTSS screening have to be in place before any waiver services get paid (DMAS CCC Plus Waiver).

Screening for CCC Plus in the community is handled by a Community Based Screening Team made up of a social worker and a Health Department nurse; if the person is hospitalized, a discharge planner can run the screening instead. Once Medicaid eligibility, screening, and waiver approval are all in place, the member can choose among waiver services and select agency-directed care, consumer-directed care, or a mix of both where those models are available (DMAS CCC Plus Waiver).

One practical distinction worth flagging: CCC Plus itself carries no waiting list, while Virginia's three separate developmental-disability waivers do.
