Ohio scrutinizes asset transfers made in the 60 months — five years — leading up to an applicant's baseline date. This look-back applies specifically to institutional and HCBS-waiver Medicaid cases built on the Special Income Level, not to MAGI-based coverage or regular ABD community Medicaid.

What counts as the baseline date depends on circumstance: usually it's the first day someone who is otherwise eligible enters an institution, or the first day they both apply for medical assistance and request enrollment in an HCBS waiver. Getting that date right matters, because it determines exactly which transfers fall inside the review window.
