To enroll in PASSPORT, an applicant needs to be at least 60 years old, meet Ohio Medicaid eligibility, require an intermediate or skilled nursing-facility level of care, live in a qualifying home or community setting, and satisfy the program's care-plan and enrollment conditions. The Ohio Home Care Waiver serves as a separate track for people who don't fit PASSPORT's profile.
