North Carolina now applies a single, uniform 60-month — five-year — look-back to transfers relevant to long-term-care Medicaid eligibility, under NCDHHS Policy MA-2240.

That uniformity is recent. A September 2025 policy revision eliminated an older 36-month exception that had applied to transfers with a starting point before November 2010, standardizing the review period across transfer types. Trusts and annuities, notably, were already subject to the full 60-month review even before that change.
