North Carolina Home Care Medicaid

North Carolina's main Medicaid home- and community-based waiver for adults is the Community Alternatives Program for Disabled Adults, known as CAP/DA, which continues to operate under fee-for-service NC Medicaid Direct rather than through managed care.

To qualify, an applicant generally needs to be at least 18, enrolled in NC Medicaid Direct under an approved long-term-care category, in need of nursing-facility level of care, assigned an available waiver slot, and covered by a person-centered plan documenting the risk of institutionalization without home- and community-based support. That slot requirement is important — CAP/DA has a limited number of openings, so eligibility alone doesn't guarantee immediate enrollment, and a waitlist is common.
