Louisiana Home Care Medicaid

Staying at home doesn't mean going without Medicaid support — Louisiana runs several Section 1915(c) HCBS waivers designed to deliver Medicaid-funded services outside of an institution. CMS's factsheet on Louisiana names the Adult Day Health Care Waiver and the Community Choices Waiver as the two aimed at people who meet a nursing-facility level of care, alongside other waivers built for different populations and care levels.

Community Choices, per CMS, serves people 65 and older along with people 21 through 64 who have physical disabilities and meet that nursing-facility level of care. Its service list covers personal assistance, nursing, home-delivered meals, adult day health care, assistive technology, temporary caregiver support, transition services, and other community-based supports.

Adult Day Health Care follows a similar pattern: CMS describes it as covering people 65 and older and people 22 through 64 with physical disabilities who meet the same level-of-care standard. Listed services include adult day health care itself, support coordination, meals, assistive technology, a personal emergency response system, and transition supports.

Access to either waiver runs through LDH's Request for Services Registry, so meeting the financial and functional criteria is only part of the picture — a request-date and priority system also determines when an offer actually comes through.
