Kentucky Home Care Medicaid

Kentucky's LTSS division runs several programs aimed at keeping people in their homes or communities rather than in an institution. Its 1915(c) home- and community-based waiver programs serve people with disabilities who need a nursing-facility level of care but want to remain outside of one, and PACE serves people 55 and older who need that same level of care while continuing to live at home.

The state also runs Kentucky Transitions, a program aimed at people moving out of a nursing home or another institution and back into their own home. That's a distinct process from waiver eligibility, but it matters for anyone already in facility care who wants a plan for returning to the community.

The practical catch is capacity: Kentucky's HCB waiver currently maintains a waitlist, and the state places completed applications by receipt date. That waitlist is a live planning constraint — meeting the financial and functional criteria doesn't guarantee an immediate service start.
