The Illinois Department of Healthcare and Family Services (HFS) administers Medical Assistance statewide, and it delivers long-term services and supports mainly through fee-for-service nursing-facility payments alongside managed care.
HFS names three distinct care-coordination programs: HealthChoice Illinois, YouthCare, and the Medicare-Medicaid Alignment Initiative (MMAI). It's worth being precise here — MMAI is its own separate demonstration program for dual-eligible individuals, not simply a subset of HealthChoice Illinois.
