Illinois Home Care Medicaid

The Illinois Department on Aging runs the Community Care Program (CCP), the state's primary in-home and community-based alternative to nursing-facility placement for seniors.

Its Medicaid-funded piece is the Persons Who Are Elderly 1915(c) waiver. HFS handles the Medicaid administration and federal oversight side of that waiver, while the Department on Aging manages its day-to-day operations — a split responsibility that's useful to understand when trying to figure out which agency to call.
