Georgia's principal route into home-based long-term care for older adults is the Elderly and Disabled Waiver Program, a federal Section 1915(c) Home and Community-Based Services waiver delivered through two separate service models: SOURCE and CCSP (Georgia waiver programs).
SOURCE stands for Service Options Using Resources in a Community Environment, and its defining feature is that it links each enrollee to a primary-care physician who helps coordinate the home- and community-based services they receive (Georgia.gov SOURCE application page).
