Connecticut Home Care Medicaid

Connecticut's main home-care vehicle for older adults is a Section 1915(c) Medicaid waiver called the Home and Community Based Services Waiver for Elders, administered through the associated state program known as the Connecticut Home Care Program for Elders — CHCPE for short. The waiver serves people age 65 and older, and its purpose is squarely to help frail elders avoid nursing-home placement or transition out of one. This is a genuine Medicaid HCBS pathway, not simply a referral service pointing families toward private home-care agencies.

Entry into CHCPE isn't automatic. DSS Community Options Unit nurses and social workers screen applicants for level of care, financial eligibility, and the program's other requirements. That means families should expect both a functional assessment and financial documentation — a physician's diagnosis alone doesn't secure a waiver slot, just as a family's care need alone doesn't establish Medicaid financial eligibility.
