CHOICES doesn't just pay for nursing-facility stays — it also funds qualifying long-term services and supports delivered in a person's home or community. Group 2 within CHOICES serves adults with physical disabilities age 21 or older, plus people 65 or older, who qualify for nursing-facility-level care but choose home- and community-based services instead.
Group 3 works differently: it's designed for adults and older people who are at risk of needing nursing-facility level of care, where receiving home care is what keeps them from reaching that point. Because slots and funding for Group 3 aren't unlimited, a home-care request should start by confirming the right clinical category rather than jumping straight to a list of desired services.
