Maryland Medical Assistance is the official name behind what most families just call "Maryland Medicaid." The program's coverage-group guide breaks long-term-care applicants into several distinct buckets: SSI recipients already receiving long-term care, an aged/blind/disabled long-term-care group, a version of that same group that qualifies through spend-down, and a separate track for people enrolled in an HCBS waiver or PACE. Which bucket someone lands in shapes the financial rules and the managed-care status that follow.

The spend-down bucket deserves special attention because it's easy to miss. Maryland's guide explicitly lists an "ABD Long Term Care With Spend-down" group, and the state's 2026 appendix schedules set medically needy income standards at $350 a month for one person and $392 for two. Those numbers aren't a nursing-home bill — they're the threshold used to work out medically needy eligibility and how much a household needs to spend down before assistance kicks in.
