# Wyoming Home Care Medicaid

Last updated 2026-10-20

The Community Choices Waiver, generally known as CCW, is Wyoming's primary Medicaid alternative to nursing-facility care for people who'd rather remain at home or in the community. The Department describes CCW as available to older adults age 65 or older who are already on, or eligible for, Medicaid, as well as adults ages 19 through 64 who have a confirmed qualifying disability. Formally, CCW is a Section 1915(c) home- and community-based services waiver, and Wyoming's own fact sheet frames it as offering a genuine alternative to nursing-facility placement through a defined set of community-based services.

It's worth understanding that CCW isn't simply regular Medicaid with a few extra hours of home care tacked on. The state's HealthStat reporting groups CCW together with nursing homes as the two routes for people who meet a nursing-home level of care — which underscores that CCW is a full long-term-care alternative, built on its own functional assessment, financial eligibility process, individualized service plan, and network of approved providers. Anyone considering this path should expect a comparable level of documentation and assessment rigor to what a nursing-facility application would require, just applied toward staying at home instead.
