# Oklahoma Medicaid Look-Back Period

Last updated 2026-10-20

For any transfer made on or after February 8, 2006, Oklahoma's ADvantage rule counts back 60 months from the first day a person is both institutionalized and has applied for SoonerCare. Under this rule, being institutionalized also covers someone already receiving ADvantage services — so waiver applicants face the same look-back exposure as nursing-home applicants. Any transfer made for less than fair value by the individual, a spouse, a guardian, or a legal representative can affect eligibility for either track.

The review casts a wide net — well beyond a single check written to an adult child. OHCA defines a transfer to include a purchase, a sale, a gift, or even a change in how an asset is titled, and it flags homes, other real property, bank accounts, CDs, and cash as common examples. The state's consumer-facing nursing-facility guidance adds a specific warning: adding another person's name to a property title can trigger a transfer issue if that person didn't pay fair market value for the share.

The look-back period runs 60 months, and the state's current published cost reference for comparison purposes is $247.72 per nursing-home day. Anyone considering a transfer, or getting ready to apply, should keep thorough transaction records and get a live agency calculation before moving forward.
