# Ohio Home Care Medicaid

Last updated 2026-10-20

Ohio offers more than one route to Medicaid-funded care outside a nursing facility. PASSPORT is the state's flagship home- and community-based waiver for older adults — the name stands for Pre-Admission Screening System Providing Options and Resources Today — jointly run by the Department of Medicaid and the Department of Aging, with Area Agencies on Aging handling local delivery.

To enroll in PASSPORT, an applicant needs to be at least 60 years old, meet Ohio Medicaid eligibility, require an intermediate or skilled nursing-facility level of care, live in a qualifying home or community setting, and satisfy the program's care-plan and enrollment conditions. The Ohio Home Care Waiver serves as a separate track for people who don't fit PASSPORT's profile.
