# New York Home Care Medicaid

Managed Long Term Care (MLTC) is the backbone of home and community-based Medicaid in New York. It's built for people who are dually eligible for Medicare and Medicaid, are 21 or older, and need more than 120 days of community-based long-term care.

Getting enrolled typically starts with a conflict-free assessment from the New York Independent Assessor, and New York Medicaid Choice is there to help applicants actually select a plan once they clear that step.

Two things to watch closely in this space: the community-care look-back (proposed at 30 months) still hasn't been implemented as of August 2026, and litigation involving the Public Partnerships LLC (PPL) fiscal intermediary system remains unresolved — both are worth checking on before making assumptions about how home care Medicaid will work in a given case.
