# Long-Term Care Funding in Missouri

Missouri does long-term-care Medicaid its own way. Rather than a hard nursing-home income cap, the state runs a spend-down structure paired with a comparatively high $6,220.50 resource limit — a combination that trips up families who assume every state works like the income-cap states nearby.

Missouri also permits a beneficiary deed that transfers a home at death, but it's not the guaranteed probate-avoidance shield it can look like on paper: Missouri elder-law sources describe cases where the state successfully brought deeded real estate back into probate to collect on a Medicaid debt. This cluster works through both of those wrinkles, plus the state's HCBS waivers and veterans benefits, using Missouri's own current figures.

## Why Missouri is different

- **Spend-down, not a hard income cap.** Missouri's July 2026 eligibility chart lets a participant above the non-spend-down income line spend down income, rather than forcing every applicant over a line into a Qualified Income Trust.
- **A high resource standard.** The individual resource maximum is $6,220.50 and the couple maximum is $12,441 — notably higher than the bare federal minimums many states use.
- **A beneficiary deed that isn't a guaranteed recovery shield.** Missouri authorizes a recorded beneficiary deed effective at death under RSMo §461.025, but a Missouri Court of Appeals decision has allowed the state to pursue deeded property by bringing it back into probate for collection.
- **Multiple HCBS waivers with different caps.** Missouri runs the Aged and Disabled, Adult Day Care, Independent Living, and Structured Family Caregiving waivers, and the Independent Living Waiver in particular has limited slots and a waitlist once they fill.
- **A narrow former-POW property-tax feature and full Aid & Attendance exclusion.** MO HealthNet excludes the verified Aid & Attendance portion of a VA pension, and Missouri operates seven state Veterans Homes.
