# Long-Term Care Funding in California

No other state has run its long-term care Medicaid program quite like California has recently. It dropped its non-MAGI asset test entirely in 2024, only to bring it back in 2026 at $130,000 for an individual and $195,000 for a couple. Layer on a 30-month transfer look-back, a standing amnesty window covering transfers made in 2024 and 2025, tightly limited SB 833 estate recovery, and the country's biggest in-home care program, and it's clear a generic Medicaid explainer won't cut it here.

Everything below reflects how the Department of Health Care Services actually administers Medi-Cal's long-term-care benefit today, pillar by pillar.

## Why California is different

- **A one-of-a-kind asset-test reversal.** California was the only state to run LTC Medi-Cal with zero asset test during 2024 and 2025, before reinstating limits of $130,000 for an individual and $195,000 for a couple on January 1, 2026.
- **A shorter, temporarily rebuilding look-back.** Its standard LTC transfer review runs 30 months rather than the 60-month federal or Florida standard, and transfers made during the permanent January 2024 through December 2025 amnesty window are never reviewed at all — the full 30-month look-back doesn't return until LTC applications or facility entries on or after July 1, 2028.
- **Estate recovery is narrow by design.** For deaths after 2016, SB 833 limits recovery to the probate estate and to long-term-care-related services, and it's completely barred if a spouse or registered domestic partner, a child under 21, or a blind or disabled child survives.
- **The nation's largest in-home care program.** In-Home Supportive Services is projected to serve 875,344 recipients in fiscal year 2026–27, dwarfing comparable programs elsewhere.
- **A different home-transfer instrument.** Instead of Florida's Lady Bird deed, California residents use the revocable transfer-on-death deed for qualifying homes, subject to its own statutory limits and formalities.

## The 10 funding pillars in California

Every pillar below has its own dedicated California page with the current rules, dollar figures, and what to do next.

- [California Medi-Cal Long-Term Care](https://fundingltcmarketplace.com/states/california/medi-cal-long-term-care/) (Medicaid Program): How California's statewide CalAIM long-term-care benefit works through Medi-Cal managed care, including the 2026 return of the non-MAGI asset test.

- [California Medi-Cal Asset Limits](https://fundingltcmarketplace.com/states/california/medi-cal-asset-limits/) (Financial Eligibility): California reinstated its Medi-Cal asset test on January 1, 2026: $130,000 for one person and $195,000 for a couple, after the unique 2024–2025 elimination.

- [California Medi-Cal Look-Back Period](https://fundingltcmarketplace.com/states/california/medi-cal-look-back-period/) (Look-Back & Penalties): California uses a 30-month LTC transfer review, with a permanent 2024–2025 transfer amnesty window and a review period that does not fully rebuild until July 2028.

- [California Transfer-on-Death Deed](https://fundingltcmarketplace.com/states/california/transfer-on-death-deed/) (Asset Protection Tool): California's revocable transfer-on-death deed is a tightly formal residential probate-avoidance tool — not a Florida-style Lady Bird deed.

- [California Medi-Cal Estate Recovery](https://fundingltcmarketplace.com/states/california/medi-cal-estate-recovery/) (Estate Recovery): SB 833 narrowed Medi-Cal recovery after 2017 deaths to probate assets and long-term-care-related services, with absolute protections for specified surviving family members.

- [California Nursing Home Medi-Cal](https://fundingltcmarketplace.com/states/california/nursing-home-medi-cal/) (Nursing Home Coverage): California nursing-home Medi-Cal uses share of cost rather than Florida's income cap, with CalAIM managed care and unusually low resident personal-needs figures reported by CANHR.

- [California Home Care Medi-Cal](https://fundingltcmarketplace.com/states/california/home-care-medi-cal/) (Home & Community Care): IHSS is California's enormous consumer-directed home-care benefit, alongside CBAS and the Assisted Living Waiver for qualifying full-scope Medi-Cal members.

- [California Long-Term Care Planning](https://fundingltcmarketplace.com/states/california/long-term-care-planning/) (Planning Ahead): California planning in 2026 means navigating reinstated asset limits, a temporarily shortened transfer review, share of cost, and probate-only estate recovery.

- [California VA Aid & Attendance](https://fundingltcmarketplace.com/states/california/va-aid-attendance/) (VA Benefits): VA Aid & Attendance has its own income, net-worth, and transfer rules, while California adds CalVet homes and a disabled-veterans property-tax exemption.

- [California Long-Term Care Insurance](https://fundingltcmarketplace.com/states/california/long-term-care-insurance/) (LTC Insurance): California regulates LTC insurance through CDI; its historic Partnership offers asset protection for existing policyholders but is currently closed to new sales.
