# Long-Term Care Funding in Alaska

Alaska funds long-term care Medicaid through an income-cap pathway paired with a Qualifying Income Trust — often called a Miller Trust — for applicants over the limit. Unlike states that publish one statewide transfer-penalty rate, Alaska calculates its penalty using the local nursing-facility Medicaid daily payment rate, so there's no single number to quote for the whole state.

Alaska Medicaid is also known as DenaliCare, and two state divisions divide the work: the Division of Public Assistance (DPA) determines financial eligibility and calculates a recipient's Cost of Care Amount, while Senior and Disabilities Services (SDS) handles the functional assessment and level-of-care determination. Alaska also has a statutory transfer-on-death deed, but its recovery statute and regulations call for a separate probate and hardship-waiver analysis rather than treating the deed as a simple planning shortcut.

## Why Alaska is different

- **Income-cap state with a required Miller Trust for excess income** — a 2026 reference reports a $2,982 monthly limit, and going over it typically means setting up a Qualifying Income Trust.
- **No single statewide transfer-penalty divisor** — the penalty period is calculated using the applicable local nursing-facility Medicaid daily payment rate rather than one number for the whole state.
- **A published home-equity statement of $500,000** — this figure comes from the Alaska Department of Health's current Medicaid application information.
- **The transfer-on-death deed doesn't touch eligibility during life** — the statute specifically states the deed does not affect public-assistance eligibility while the owner is alive, but recovery after death is a separate question.
- **Estate recovery includes a pre-death lien option** — beyond the standard post-death claim, the state can place a lien on a still-living recipient's property under specific institutional and hearing conditions.
- **Alaska is generally treated as a non-Partnership state** — current consumer references identify Alaska as outside the dollar-for-dollar Partnership asset-protection framework that some other states offer.

## The 10 funding pillars in Alaska

Every pillar below has its own dedicated Alaska page with the current rules, dollar figures, and what to do next.

- [Alaska Medicaid Asset Limits](https://fundingltcmarketplace.com/states/alaska/medicaid-asset-limits/) (Financial Eligibility): Alaska's reported 2026 LTC resource and income limits, community-spouse protection, $500,000 current home-equity statement, and QIT rule.

- [Alaska Medicaid Look-Back Period](https://fundingltcmarketplace.com/states/alaska/medicaid-look-back-period/) (Look-Back & Penalties): Alaska reviews five years of transfers and uses the local nursing-facility payment rate, not one statewide published divisor.

- [Alaska Transfer-on-Death Deed](https://fundingltcmarketplace.com/states/alaska/transfer-on-death-deed/) (Asset Protection Tool): Alaska's recorded, revocable TOD deed does not convey a beneficiary interest during life and needs separate Medicaid transfer and recovery review.

- [Alaska Medicaid Estate Recovery](https://fundingltcmarketplace.com/states/alaska/medicaid-estate-recovery/) (Estate Recovery): Alaska estate recovery includes statutory family protections and an undue-hardship process with a 30-day appeal deadline.

- [Alaska Nursing Home Medicaid](https://fundingltcmarketplace.com/states/alaska/nursing-home-medicaid/) (Nursing Home Coverage): Alaska Nursing Home Medicaid combines DPA financial eligibility, an SDS nursing-facility level-of-care determination, and a $200 monthly personal-needs allowance.

- [Alaska Home Care Medicaid](https://fundingltcmarketplace.com/states/alaska/home-care-medicaid/) (Home & Community Care): Alaska operates five HCBS waivers, including ALI for older adults, while its IDD and ISW pathways have stated waitlist or participant-limit rules.

- [Alaska Long-Term Care Planning](https://fundingltcmarketplace.com/states/alaska/long-term-care-planning/) (Planning Ahead): Alaska planning must coordinate a Miller Trust, Native-property rules, the five-year transfer review, locally variable penalty rates, and TOD deed analysis.

- [Alaska VA Aid & Attendance](https://fundingltcmarketplace.com/states/alaska/va-aid-attendance/) (VA Benefits): Federal Aid and Attendance can supplement a qualifying pension; Alaska has one State Veterans Home in Palmer and a disabled-veteran property-tax exemption framework.

- [Alaska Long-Term Care Insurance](https://fundingltcmarketplace.com/states/alaska/long-term-care-insurance/) (LTC Insurance): Alaska's Division of Insurance regulates LTC insurance, but current consumer references identify Alaska as a non-Partnership state rather than an open asset-protection market.
